ENROLLMENT PROCEDURE: COLLEGE (NEW STUDENT)

PROCEDURE

STEP 1: ADMISSIONS OFFICE

Fill out Admission Forms

Submit the following requirements:

  • Grade 12 Report Card /School Form 9 (Original)
  • Certificate of Good Moral Character (Original)
  • 1 pc Recent 1x1 ID Photo 
  • PSA Certified Birth Certificate (Photocopy)
  • Accomplished Transferee Processing Form (for Transferee) 
  • Transfer Credential / Honorable Dismissal (for Transferee)
  • TOR/List of Subjects taken (for Transferee)  
  • 4th Year Report Card (for OBEC Graduate)
  • Certificate of Grades (for ALS Graduate)

STEP 2: DEAN’S OFFICE

  • Subject Enlistment 

STEP 3: REGISTRAR’S OFFICE

  • Claim the Assessment of Fees

STEP 4: FINANCE OFFICE

  • Payment of Tuition Fee

STEP 5: REGISTRAR’S OFFICE 

  • Claim the Printed Registration Form 

 

Receive an email with Canvas credentials.

Note: Only students who PAID the required fees and completed the above steps will be considered ENROLLED
